Buprenorphine for the management of kratom dependency during covid-19: A case report

چکیده
We describe a case report where buprenorphine was successfully used for the management of kratom ( Mitragyna speciosa) dependency during Covid-19 pandemic in specialist community substance misuse treatment service. The individual had notable iatrogenic on oral over counter and prescribed opioid medication. She resorted to using as ‘tea’ self-detoxify before getting into difficulty. Upon presentation, she disclosed up 70 g/day with hourly dosing, addition co-codamol (8 mg codeine/500 paracetamol tablets) three times week. Initiation sublingual tablets complicated due Covid-19: dispensing arrangements were liberalised from outset dose incrementally increased, eventually 24 mg/day. Comorbidities included mental health conditions chronic pain which improved psychotherapy. Further research is required determine optimal regimens dependency. This study adds current paucity published data regarding how manage such cases setting, important since thought be increasingly western world.
